Please bring the following items with you to the recording department:

1. Old passport OR Certificate of Naturalization OR certified copy of birth certificate from the state board of health with a seal



2. Two regulation passport photographs 2" x 2". Pictures may be taken in the Clerk's Office for $10.70. The LAST pictures and applications will be taken at 4:15 PM



3. Valid drivers license or state issued identification card



4. FEES:

   $25 for the Clerk of the Circuit Courts fee, payable by cash or Florida check.
   
   AND individual payment for each application:
   
   Adult: $75 for the U.S. Department of State, payable ONLY by check or money order
   
   Child under 16 years of age: $60 for the U.S. Department of State, payable ONLY by check or money order
   
   

NOTE: Children must be present when applying for a passport. Both parents must sign for children 15 and under and present a long form birth certificate with both parents names.

Passports are issued by the U.S. Department of State and not the Office of the Clerk of Circuit Court. The majority of passports are sent to the Miami Passport Office for processing.
